    Análisis de la eficiencia del gasto municipal y de sus determinantes. DT. 260

    In this study we analyzed the efficiency of the spending of 1686 Peruvian municipalities for the year 2003 through the evaluation to the municipal public activities as a process of production which transforms inputs in outputs (Bradford et. al., 1969 y Fisher, 1996). In doing so, we established several “best -practice” production frontiers in order to estimate the extent of municipal spending that seems to be wasted relative to that frontier. Five methodologies for the production frontier estimations were used (i) 3 non parametric: (i) Free Disposal Hull (FDH) and Data Envelopment Analysis (DEA-CRS and DEA-VRS) and (ii) 2 parametric: one deterministic and the other stochastic, which were estimated through 10 categories of municipalities (4 groups of provincial and 6 of district municipalities) defined through a cluster methodology. Finally, using TOBIT regression models, we analyzed the fiscal, socioeconomics and demographic determinants of the efficiency levels. We found different levels of efficiency and although we identified some good practices, the result at the national level is matter of concern because the same results could have been obtained with 57.6% less resources. Among the main determinants of this inefficiency spending the FONCOMUN and canon transfers were found, mainly at the district municipal level. One of the factors that allowed for a more efficient provision of local services was the civil participation, represented in the study by the Local Coordination Council. The results of the study show that there is a need to make more efforts to improve the local government spending. Even more so, after the promulgation of the Supreme Decree No. 068-2006-PCM in October 2006, by virtue of which the culmination of the competences and function transfers process, initially programmed for 2006-20010, brought forward to the end of 2007.

    El grado de orientación pro-pobre de las políticas económicas peruanas: una revisión bibliográfica

    This paper is a balance of the Peruvian economic policies, taken by government in the last decade. We focus on determining, based on the available literature, whether or not these polices have improved the economic well-being of poor people. Furthermore, this balance allow us to give the agenda for future research on these areas, as well as to shed some light about the pro-poor necessary policies. There are several policies that have been taken into account in this balance. Macroeconomic policies; trade and investment policies; specific sector promotion policies (agriculture, industry and mining sector); labor (both formal and informal), capital and land markets; regulation of goods and services markets (telecommunications and electricity); taxation policies; public expenditure; security and risk management mechanisms and, finally, regional policies.

    Empoderamiento de la mujer y demanda por servicios de salud preventivos y de salud reproductiva en el Perú 2015-2018

    En el mundo y en el Perú se ha puesto en primer plano de la agenda la Universalización de la Salud, siendo un área específica de preocupación las necesidades de salud sexual y reproductiva. En el Perú el 85% de las mujeres unidas no desea más hijos o los quiere postergar por 2 o más años, pero apenas 55% usa métodos anticonceptivos modernos, y se calcula en medio millón las mujeres con necesidades insatisfechas de planificación familiar, lo que significa que están en riesgo de quedar embarazadas cuando no lo desean. Un problema central para enfrentar estas necesidades de salud es la debilidad de la demanda efectiva, es decir, las familias y personas en muchos casos no tienen conocimiento, no han internalizado ni tienen como práctica usual el procurar esas atenciones. Nuestra investigación analiza el efecto que tiene el empoderamiento de las mujeres sobre la demanda de atenciones de salud sexual como el uso de métodos anticonceptivos, el despistaje de VIH/SIDA y las consultas ante síntomas básicos de infecciones de trasmisión sexual. Usamos la encuesta ENDES del periodo 2015-2018. Medimos el empoderamiento de la mujer utilizando las respuestas directas de las mujeres, agrupándolas en 4 grandes categorías: decisión sobre las compras del hogar, libertad de movimiento, respeto de la pareja hacia sus deseos y derechos, y autonomía para tomar decisiones sobre su propia salud. Encontramos que el indicador de decisión sobre compras del hogar no resultó significativo o con los signos esperados. La libertad de movimiento de la mujer a un nivel medio incrementa la probabilidad de uso de métodos anticonceptivos modernos y de métodos de decisión de la mujer mas no aumenta el uso de los que son de decisión de la pareja. El respeto de la pareja hacia los deseos y derechos de la mujer influye positivamente en el uso de métodos anticonceptivos modernos y en los de uso de la pareja masculina en particular. El que la mujer tenga mayor autonomía en tomar decisiones respecto de su propia salud resulta el indicador de empoderamiento con resultados más amplios sobre el uso de servicios de salud sexual y reproductiva, afectándolos positivamente con excepción del despistaje de VIH/SIDA. Una mirada general muestra que diversos indicadores de empoderamiento de la mujer sí tienen efectos positivos y significativos sobre la demanda por métodos anticonceptivos y servicios de salud sexual y reproductiva. Los efectos son variados, con relaciones diversas entre los distintos ámbitos de empoderamiento de la mujer y los diversos métodos y servicios de salud sexual y reproductiva.

    Bonos’: Lecciones de las transferencias monetarias no condicionadas durante la pandemia de COVID-19 en Perú

    La pandemia de Covid-19 fue particularmente severa en Perú, y junto con las medidas de cuarentena, produjo un choque muy grande en las economías de los hogares. Las transferencias monetarias no condicionadas, conocidas popularmente en Perú como ´bonos´, fueron la principal respuesta de protección social del gobierno, con grandes implicaciones para la evolución de la pandemia, la economía y la pobreza. Estas transferencias se dieron en un contexto en el que se necesitaba una respuesta rápida y masiva al shock sanitario, social y económico de la pandemia, partiendo de una red de seguridad social bastante limitada a pesar del buen desempeño macroeconómico y macrofiscal del Perú. Nuestro análisis muestra que estas transferencias incondicionales supusieron un cambio significativo en la política de protección social, pero tardaron mucho tiempo en realizarse y su cuantía fue demasiado baja, lo que limitó su impacto. Las principales lecciones son que: i) la baja densidad de la red de protección social peruana dificultó una respuesta adecuada a la emergencia sanitaria y social, ii) la definición de estrategias de focalización vs. universalización en protección social debe basarse en objetivos más amplios que la mera reducción de la pobreza, incluir los objetivos de salud y gobernabilidad democrática, y debe situarse en el contexto amplio del impacto de las diversas políticas de gasto público e ingresos en toda la distribución del ingreso, iii) una vez definida la orientación hacia políticas más universalistas, fue posible avanzar rápidamente, en ausencia de restricciones fiscales significativas y aprovechando las capacidades existentes de diversos organismos públicos y de la red del sistema financiero.

    Derecho a la salud, cáncer y política fiscal en el Perú

    El Perú registra grandes desigualdades sociales y un bajo gasto público en salud. Ha avanzado en enfermedades trasmisibles y salud materno-infantil, pero las no trasmisibles hoy constituyen tres quintas partes de la carga de enfermedad y hay 45 mil nuevos casos de cáncer al año. La baja inversión social y centralización de la atención del cáncer discriminan contra los indígenas. Un tercio de limeñas ha hecho mamografía en los últimos 2 años pero apenas 4% en zonas rurales. La cobertura del Papanicolaou, un examen preventivo básico, llega solo a 50% en sierra y selva. En Lima se concentran casi tres de cada cuatro de los especialistas y hay un alto costo del tratamiento. Habiendo gozado el Perú del boom de las materias primas, el Estado ha actuado de manera tardía e insuficiente ante el cáncer. Recién a partir de 2012, luego de 8 años de auge fiscal, se estableció un programa público de prevención y control. El cáncer da cuenta del 18% de las muertes en el Perú, pero menos del 6% presupuesto en salud, apenas 0,12% del PBI. Mientras entre 2008 y 2012 la recaudación tributaria aumentó de USD 13 mil a 26 mil millones anuales, en 2012 el programa contra el cáncer solo recibía 130 millones anuales. Desde el 2014 la recaudación tributaria ha caído y el gobierno ha frenado el gasto en salud y en especial en cáncer, que ha registrado reducciones presupuestales el 2016 y 2017. Peru is a country of great social inequalities and low public expenditure in health. There have been advances in communicable diseases and maternal and infant health, but non-communicable diseases constitute 60 per cent of the burden of disease and there are 45 000 new cancer cases per year. Low social investment and a centralized system of cancer care discriminate against indigenous people. One third of women en Lima have had a mammography in the last two years but only 4 per cent in total areas. Papanicolau, a basic preventive exam for cervix cancer, covers only 50% of women in the Andean and jungle regions. Lima concentrates three of every four cancer specialists and the costs of treatment are high. Having Peru benefit from the commodities prices boom in the last decade, the policy against cancer has been slow and insufficient. Only from 2012, after eight years of fiscal plenty, a public program against cancer was established. Cancer accounts for 18% of all deaths in Peru but less than 6%of the national health budget, equivalent to only 0,12% of GDP. Between 2008 and 2012 fiscal revenues increased from USD 13,000 to 26,000 million, however, in 2012 the budget for cancer was only USD 130 million. Since 2014 fiscal revenues have gone down and the government has reduced the budget for cancer in 2016 and 2017.

    El derecho a la educación intercultural bilingüe y la política fiscal en Perú

    Para la educación peruana, el problema de la calidad aparece hoy como el mayor problema, habiéndose logrado niveles de cobertura de primaria ya casi universales al año 2000. En las escuelas EIB (ubicadas en zonas indígenas y cuyos docentes debieran enseñar usando también la lengua originaria) solo el 26,5% de estos estudiantes obtuvieron el nivel de satisfactorio en comprensión lectora; existe una brecha enorme de 23 puntos en relación al resto del país. Es necesaria una pedagogía bilingüe, que respete su propia cultura y entable un diálogo con ella. La educación intercultural bilingüe ha tenidos avances muy tardíos y limitados en el quinquenio pasado, con producción de materiales educativos en lenguas nativas, acompañamiento pedagógico en escuelas EIB, becas para la formación docente en EIB y un Plan Nacional. Consolidado el Perú como un país de ingresos medios-altos, habiendo gozado de una década de fuerte crecimiento económico y auge fiscal gracias al boom de las materias primas, el Estado ha destinado recursos al derecho a la educación de manera muy insuficiente. Durante el periodo de “vacas gordas” que tuvo la economía peruana, del 2004 al 2014, apenas 14% del gasto adicional realizado por el Estado se destinó a educación. Para la Educación Intercultural Bilingüe el tratamiento presupuestal fue discriminatorio en extremo, con apenas el 0,6% del presupuesto en educación y el 0,1% del gasto público total el 2016, en un país donde entre un sexto y una cuarta parte de la población es indígena. The main problem of education in Peru is the quality of it, having achieved almost universal coverage of primary education by 2000. In intercultural bilingual schools (localized in indigenous areas and whose teachers should use native languages) only 26,5% of its students achieved a satisfactory level in reading comprehension, 23 percent less than the rest of the children. A bilingual pedagogy is necessary that respects its own culture and engages in a dialogue with it. In Peru, the intercultural bilingual education has had a late and limited development in the last five-year period, with the production of educational materials in native languages, pedagogical advice in bilingual schools, scholarships for those who pursue studies in bilingual intercultural education and the formulation of a National Plan. Peru has established itself as a high-medium income country after a decade of rapid economic growth and fiscal boom thanks to the high prices of commodities. However, insufficient resources have been oriented towards education. During the times of plenty 2004-2014, only 14% of the additional public expenditures where spent in education. For Intercultural Bilingual Education, the budget was very small: only 0,6% of the education budget and 0,1% of total public budget in 2016, in a country where between one quarter and one sixth of the population in indigenous.

    Políticas sociales: balance y propuestas

    An evaluation of the social development of Peru at year 2000, shows that there has been improvements in several areas, such as the coverage of basic health services, but that little has been accomplished in poverty reduction and the quality of education remains poor. Economic policies haven’t promoted a fast and labor-intensive growth nor have allowed an increase in public expenditures. Reforms in social policies have been partially implemented, and an evaluation show its limits in solving problems such as reduced coverage of social security and inequality. The second part of the paper is dedicated to policy proposals. In the first term, there are proposals related to social policies and political relations: establishing effective rights for the people, democratizing policies and forging a national consensus for an increase in social expenditures. About social policies themselves, it is proposed that an integrated approach should be considered. Priorities should be basic, poor people and rural areas. Decentralization, improving capabilities of social workers (teachers, physicians) and establishing a modern social management are other policies to be implemented.

    Focalización del gasto público en salud en el Perú : situación y alternativas

    The targeting of public health expenditures in Peru is analyzed in a double way. In the first part, an analysis of the actual distribution of public health expenditures, between social strata and between regions, shows that it this expenditures are distributed inequitably. Users’ fees do not concentrate in those who have more capacity to pay. The second part proposes a multiple-way strategy for improving the targeting of the public health expenditures, including giving priority of maternal and child health care, the adaptation of these services to the cultural requirements of indigenous people, a gradual geographic reallocation of public health funds towards poorer and needier regions, the improvement of self-selection promoting private services and the individual identification of the poorest by “proxy-means test” methods in some areas.

    El cobro de tarifas y la equidad en la distribución del subsidio público en salud en el Perú

    The effect of cost recovery in the equity of the services of the Ministry of Health of Peru in health is analyzed. The funds obtained by this way reach 15 % of the health sector budget, which compares with 7% in average in developing countries; some hospitals obtain so much funds from fees charged to the public as from the treasury. It is also found that exemptions of the payment of fees is not given according to standards that adequately identify the poor, that a small percentage of the poor is exempted -undercoverage of exemptions- and that there are high levels of filtrations -non-poor exempted-. Finally, a global analysis shows that fees’ payments do not have a progressive effect over the distribution of the public health subsidy.

    Impacto económico de la malaria en el Perú

    Malaria is one of the more important public health problems in Peru and the world. Besides the human costs, that are the most important, malaria has an economic impact. This paper estimates the cost that malaria has for the Peruvian society in 119.5 million of Soles in 1998, equal to 1.5 per cent of the total health expenditures. Because these costs are principally borned by the families, that try to recuperate them by additional work efforts, they are not well perceived by the society and the disease is subvalorated in public policies.
