    THE KEY ROLE OF COMMUNITY HEALTH WORKERS IN THE USE OF MATERNAL SERVICES IN RURAL SETTINGS: A QUALITATIVE STUDY

    OBJECTIVE.: To understand the perception of the role of Community Health Workers (CHWs) as facilitators of the use of maternal health services in rural settings. Materials and methods. Semi-structured interviews were conducted with 11 CHWs assigned to the Paucara Health Center, selected by theoretical saturation sampling. Data were analyzed using a phenomenological approach, and constant comparison was used to inductively identify emerging themes. RESULTS.: The interviews revealed three categories of perceptions: 1. Accompaniment and monitoring: pregnancy identification, home visits, community awareness, transfers in emergency cases, and challenges such as lack of remuneration and community misunderstandings; 2. Cultural and institutional mediation: integration of traditional practices, respectful treatment, addressing cultural barriers; 3. Management: reporting the situation of pregnant women, training, and communal referral. CONCLUSIONS.: CHWs perceive their roles as relevant in mitigating social, cultural, and structural barriers in maternal care. This study provides evidence for the design of public policies that can strengthen the recognition and sustainability of the CHWs' work, through an equitable and culturally sensitive maternal care model.

    Navigating strategies for intercultural maternal and newborn care in Latin America and the Caribbean: a scoping review

    In Latin America and the Caribbean (LAC), women from racial and ethnic minority groups experience disproportionately worse maternal health outcomes. In addition to facing barriers to accessing maternity services, they often receive culturally inappropriate care. To address these inequities, a range of intercultural approaches to maternal and newborn care has been implemented across the region. However, the extent and nature of these initiatives over the past decade remain underexplored. We conducted a scoping review to identify, categorize, and describe strategies for promoting intercultural maternal and newborn care in LAC. We searched nine databases and grey literature for studies published in English, Spanish, or Portuguese between 2013 and 2024. This review identified 75 studies and 16 strategy categories across 13 countries. Most studies addressed Indigenous populations and employed qualitative methods. Strategies were grouped into four cross-cutting domains: (i) healthcare adaptations and workforce capacity building, (ii) intercultural communication and knowledge exchange, (iii) community engagement, and (iv) institutional support and governance. The identified strategies engaged stakeholders at the community, facility, and system levels. Based on these findings, we developed a framework for interculturality in maternal and newborn care. Our analysis indicates that sustainable implementation depends on addressing structural barriers and integrating intercultural principles into health system planning, service delivery, policy, and governance. The proposed framework can inform research, policymaking, and development of intercultural maternal and newborn care initiatives in LAC and other culturally diverse settings.

    Agencia para la ampliación de la capacidad financiera de las mujeres: Estudio cualitativo en dos distritos de Lima Metropolitana, Perú

    Las brechas de inclusión financiera en Perú limitan las capacidades de ahorro de las mujeres que viven en Lima Metropolitana, específicamente en los distritos de Comas y Carabayllo. Este estudio examina cómo la agencia, entendida como la capacidad de establecer metas, percibir control y actuar en consecuencia, se relaciona con la capacidad financiera (ahorro) en las mujeres. Mediante entrevistas cualitativas a 15 mujeres entre 21 y 65 años, se encontró que la agencia resulta central para la capacidad financiera y viceversa. Tan importante como tener conocimientos sobre productos o servicios financieros formales e informales, es la capacidad que tienen las mujeres para establecer metas y su sentido de control sobre los recursos y los resultados. El ahorro va más allá de la acumulación material, siendo esta una expresión de la agencia que concilia metas personales con la sostenibilidad de la vida familiar. La capacidad de ahorro se encuentra condicionada por la disponibilidad de acceso a recursos financieros (servicios y productos formales o informales), la feminización del trabajo de cuidados no remunerado, la informalidad y contingencias externas en contextos de alta vulnerabilidad económica, que requieren ser atendidas, a través de políticas públicas, para garantizar que la ampliación de la capacidad de ahorro contribuya con mayores niveles de autonomía económica y libertad de las mujeres.
